nursing resignation letter? This is what I have written so far...?

Please accept my resignation as a Licensed Practical Nurse from xyz health center effective Wednesday, July 23, 2008. Working for xyz has been a wonderful experience. I could not ask for a better group of colleagues. I have grown in many ways here and will always treasure the opportunities provided for me by xyz. Due to the demands of the RN Nursing program I need to devote my time to my studies. If you have any questions, please feel free to ask. Thank You

2. you only need the first line. Please accept my resignation as a Licensed Practical Nurse from xyz health center effective Wednesday, July 23, 2008. you don't need to say why you are leaving or that you liked it that. that is all personal information. sign and date it and you are done.
3. I agree that you only need the first line. Remember they will be keeping this on file for years, maybe forever. Tell them your plans in person at the time you give them the paper if you think it's necessary. But don't write it.
